By completing this application, I hereby apply for membership in the Houston Tennis Umpires Association. I agree to abide by all the Rules and Regulations set forth in the HTUA Constitution
and By-Laws.

I hereby acknowledge that I am a contract worker for the HTUA and cannot hold the HTUA liable for any claims of unemployment, loss or injuries which I may suffer or sustain by me in connection with my activities during the period of official assignment or traveling to and from the event. All claims are hereby waived and released and I covenant not to sue therefore.
